Auf einem normalen EKG sind drei Zacken sichtbar. Die P-Welle entspricht der Erregung (Kontraktion) der beiden Vorhöfe der Herzens. Die QRS-Zacke entspricht der Erregung der Herzkammern und entsteht bei der Anspannung der Kammern.

Ein typisches Zeichen, das auf einen Herzinfarkt hinweist, sind Veränderungen des EKGs im Bereich der ST-Strecke. Ist die ST-Strecke im Vergleich zur Grundlinie angehoben (s. Abb. unten: ↑ Symbol) spricht man von einer ST-Streckenhebung (kurz ST-Hebung) und einem STEMI (ST-Hebungs-Myokardinfarkt).
